We report the identification of a recurrent, 520-kb 16p12.1 microdeletion associated with childhood developmental delay. The microdeletion was detected in 20 of 11,873 cases compared with 2 of 8,540 controls and replicated in a second series of 22 of 9,254 cases compared with 6 of 6,299 controls. Most deletions were inherited, with carrier parents likely to manifest neuropsychiatric phenotypes compared to non-carrier parents. Probands were more likely to carry an additional large copy-number variant when compared to matched controls. The clinical (...) features of individuals with two mutations were distinct from and/or more severe than those of individuals carrying only the co-occurring mutation. Our data support a two-hit model in which the 16p12.1 microdeletion both predisposes to neuropsychiatric phenotypes as a single event and exacerbates neurodevelopmental phenotypes in association with other large deletions or duplications. Analysis of other microdeletions with variable expressivity indicates that this two-hit model might be more generally applicable to neuropsychiatric disease. © 2010 Nature America, Inc. Both arguments conclude that theists have an epistemic advantage over atheists/naturalists vis-à-vis the question whether or not our cognitive faculties are reliable. In this paper, I show how these arguments bear an even deeper resemblance to each other. After bringing the problem of evil to bear negatively on Descartes’s argument, I argue that, given these similarities, atheists can wield a (...) recent solution to the problem of evil against theism in much the way Plantinga wields the detailsof evolutionary theory against naturalism. I conclude that Plantinga and Descartes give us insufficient reason for thinking theists are in a better epistemic position than atheists and naturalists vis-à-vis the question whether or not our cognitive faculties are reliable. Discussions of science and values in risk management have largely focused on how values enter into arguments about risks, that is, issues of acceptable risk. Instead this volume concentrates on how values enter into collecting, interpreting, communicating, and evaluating the evidence of risks, that is, issues of the acceptability of evidence of risk. By focusing on acceptable evidence, this volume avoids two barriers to progress. One barrier assumes that evidence of risk is largely a matter of objective scientific data and (...) therefore uncontroversial. The other assumes that evidence of risk, being "just" a matter of values, is not amenable to reasoned critique. Denying both extremes, this volume argues for a more constructive conclusion: understanding the interrelations of scientific and value issues enables a critical scrutiny of risk assessments and better public deliberation about social choices. The contributors, distinguished philosophers, policy analysts, and natural and social scientists, analyze environmental and medical controversies, and assumptions underlying views about risk assessment and the scientific and statistical models used in risk management. Central to Perelman and Olbrechts-Tyteca’s philosophical revival of rhetoric and dialectic is the importance given to the temporal character of argumentation. Unlike demonstration, situated within the “empty time” of a single instant, the authors of The New Rhetoric understand argumentation as an action that unfolds within the “full time” of meaningful human life. By taking a broader view of his work beyond The New Rhetoric, I first outline Perelman’s understanding of time and temporality and the challenge that it poses for (...) the study of argumentation. Next, I emphasize the distinction between argumentation’s internal and external temporal structures, and then show how Perelman problematizes a static view of a number of basic argumentative concepts by bringing out their essentially temporal character. Finally, in clarifying what is at stake in Perelman’s account, I conclude by drawing attention to a number of issues in contemporary argumentation studies that may benefit from a reconsideration of Perelman’s analysis of time. It is widely believed that Perelman and Olbrechts-Tyteca’s theory of argumentation is vulnerable to the charge of relativism. This paper provides a more charitable interpretation of Perelman and Olbrechts-Tyteca’s normative views, one that properly considers the historical trajectory of their work and a wider range of texts than existing interpretations. It is argued that their views are better characterized as a form of “contrastivism about arguments” than any kind relativism. This more accurate depiction contributes to ongoing efforts to revive interest (...) in Perelman and Olbrechts-Tyteca’s work as well as build bridges with trends in contemporary argumentation theory. This paper re-examines the well-known problem of how it is possible to have an “intuition of absences” in Sartre’s example of Pierre. I argue that this problem is symptomatic of an overly theoretical interpretation of Sartre’s use of intentionality. First, I review Husserl’s notion of evidence within his phenomenology. Next, I introduce Sartre’s Pierre example and highlight some difficulties with interpreting it as a problem of perception. By focusing on Sartre’s notion of the project, I argue instead that the problem (...) is better understood at the level of action. In support of this interpretation, I conclude with a brief comparison to the early work of Paul Ricoeur. By borrowing some of Ricoeur’s phenomenological vocabulary tailored to action, I reinterpret Sartre’s example as a practical problem. (shrink)

Firearm injury in the United States is a public health crisis in which physicians are uniquely situated to intervene. However, their ability to mitigate harm is limited by a complex array of laws and regulations that shape their role in firearm injury prevention. This piece uses four clinical scenarios to illustrate how these laws and regulations impact physician practice, including patient counseling, injury reporting, and the use of court orders and involuntary holds. Unintended consequences on clinical practice of laws intended (...) to reduce firearm injury are also discussed. Lessons drawn from these cases suggest that physicians require more nuanced education on this topic, and that policymakers should consult front-line healthcare providers when designing firearm policies. Indifference, Necessity, and Descartes's Derivation of the Laws of Motion BLAKE D. DUTTON WHILE WORKING ON Le Monde, his first comprehensive scientific treatise, Des- cartes writes the following to Mersenne: "I think that all those to whom God has given the use of this reason have an obligation to employ it principally in the endeavor to know him and to know themselves. This is the task with which I began my studies; and I can say that I would not (...) have been able to discover the foundations of physics if I had not looked for them along that road" . ' As the letter makes clear, knowl- edge of the foundations of Cartesian physics is inextricably linked to the knowledge of God. Unfortunately, Descartes never explains why this is the case, and the relation in which his theistic doctrine stands to his physics remains unspecified. In what follows I wish to clarify that relation by examining some of the problems surrounding Descartes's attempt to locate the metaphysical founda- tions of his physics in his doctrine of God. I begin my analysis with a discussion of the doctrine of divine indifference and argue that this doctrine is of great importance to any interpretation of the foundations of Cartesian physics, insofar as it provides Descartes with a rationale for dismissing the appeal to final causation in scientific explanation. In this paper I trace Ricoeur’s reflections on ideology and utopia from his Lectures on Ideology and Utopia, first delivered in 1975, to his later writings on selfhood and the just from the 1990s. The thread that I follow begins from the closing lines of Ricoeur’s Lectures, wherein he suggests that “practical wisdom” (or phronesis) may provide an answer to the paradox of ideology. Taking this suggestion as my point of departure, I reread Ricoeur’s earlier solution to this problem back (...) from the vantage point of his later writings, where his conception of phronesis is further developed. Although these later writings are not immediately concerned with ideology, Ricoeur’s idea of “phronetic judgment” can still be understood within the earlier problematic. As I argue, Ricoeur’s concept of phronetic judgment helps to consolidate his earlier solution to the problem of ideology within his later, more systematic reflections on ethics, politics, and practical philosophy. Although Ricoeur’s reflections on ideology and utopia have been subject to considerable scrutiny, commentators typically discuss them within the context of his writings from the same period. The longer view that I adopt here therefore not only sheds light on questions of continuity in Ricoeur’s political thought, but may also stimulate further interest in his contribution to ideology critique and contemporary critical theory more broadly. This paper argues that Ricœur’s philosophy operates on the basis of a more expansive conception of rhetoric than it first appears. To show this, I reread The Rule of Metaphor through the “new rhetoric” of Chaïm Perelman. First, I survey Ricœur’s understanding of rhetoric in the 1950s and 60s. Second, I examine Ricœur’s relation to Perelman within the context of the broader “rhetorical turn” of the 1970s. After examining their respective positions, I argue that Ricœur fails to appreciate the full (...) significance of Perelman’s conception of audience. In doing so, I draw attention to the central role that Ricœur himself ascribes to the audience or reader in the “work of meaning.” I conclude by proposing that the rhetorical triad of logos/ethos/pathos may serve as a conceptual matrix with which the rhetorical aspects of Ricœur’s philosophy can be interpreted. It is considered the right of children to have their pain managed effectively. Yet, despite extensive research findings, policy guidelines and practice standard recommendations for the optimal management of paediatric pain, clinical practices remain inadequate. Empirical evidence definitively shows that unrelieved pain in children has only harmful consequences, with no benefits. Contributing factors identified in this undermanaged pain include the significant role of nurses. Nursing attitudes and beliefs about children’s pain experiences, the relationships nurses share with children who are suffering, (...) and knowledge deficits in pain management practices are all shown to impact unresolved pain in children. In this article, a relational ethics perspective is used to explore the need for nurses to engage in authentic relationships with children who are experiencing pain, and to use evidence-based practices to manage that pain in order for this indefensible suffering of children to end. This position, of course, was in direct opposition to the prevailing accounts of causality within the philosophical tradition of Islam at the time. The philosophers (falāasifa) had by and large taken over accounts of causality from Aristotle and the Neoplatonists and adapted them in accordance with their own set of concerns. In such accounts, while God stands as the first cause, secondary causation—the causative action of agents other than God—is unambiguously affirmed, even if variously understood. Mais nous ne l’aurions problement pas eue sous la forme volumineuse qu’elle a prise avec toutes ces Objections et Reponses, qui font plus que (...) quintupler les Méditations primitives.1(AT XII, 306)While not charging Descartes with dissimulation, Adam argues that the metaphysics was developed to deflect attention from the heart of the Cartesian system—its mechanistic physics—which posed a direct challenge to the reigning Aristotelianism. In his words, it served primarily as a “tent... to cover the merchandise (le pavilion... couvrir la marchandise)”(AT XII, 306).2Adam is certainly not alone in pointing to the Galileo affair as grounds for raising suspicions about the role of the metaphysics in Descartes’ larger work. In a recent biography, itself destined to become a classic, Stephen Gaukroger takes a similar line, arguing that it was Galileo’s failure to convince his opponents through arguments drawn from natural philosophy that prompted Descartes to seek acceptance of his own natural philosophy though metaphysical legitimation. Had Galileo not been silenced there would have been no impetus for Descartes to have developed a metaphysical argument; natural-philosophical [End Page 49] arguments would have sufficed.3 Thus, in the words of Gaukroger, Descartes’ project of metaphysical legitimation was “a task which he never even contemplated before the condemnation of Galileo in 1633, and... was a direct response to that condemnation.”4 If we are to believe Adam and Gaukroger, then, Descartes was first and foremost a natural philosopher whose extended venture into metaphysics was primarily a strategic response to the events of 1633.Despite the many merits of this line of interpretation, I believe it is trouble-some insofar as it isolates Descartes’ reaction to the condemnation from both his criticisms of Galileo’s science and his views on the relation between science and theology. In doing so, it overstates the degree to which Cartesian science can itself be isolated from Cartesian metaphysics. In what follows, then, I will contextualize Descartes’ reaction to the Galileo affair by examining his comments on Galileo’s scientific work and the relation of his own views on science and theology to those of Galileo. Two results will emerge. First, we will see that if we fail to take Descartes’ search for metaphysical foundations as having a genuinely scientific motivation, not only will we render his criticisms of Galileo’s science unintelligible, but we will fail to understand the distinctive nature of his science vis-à-vis Galileo’s. This will become clear as we examine the work of each on motion and the fall of bodies.
